FINAL YEAR = 40 WEEKS OF PREGNANCY

Every final year student male or female goes through the same joys and pains of any pregnant woman. Its those final 9months and you are out, as a “doc”
Changes during pregnancy
First trimester-
The 1st day of final year is very exciting- 1 last year of MBBS and 4 new much more interesting subjects- just as excited as a woman who gets to know she is pregnant. :-)
But with it obviously come the not so exciting things-
1)Amenorrhea- No more 4-5 days of outing and tp, every month for next 9 months
2)Morning sickness- You get up every morning and the same thought daily, Oh god, postings today, have to reach at 9am
3)Fatigue-It’s the 1st few months, obviously not used to the hectic schedule
4)Weight gain- My my, its already 1 kg now, have 10 kgs to add on the remaining months
5)Increase in frequency of micturition- That’s the only place you can probably go, to escape from everything
6)Per abdominal and pelvic changes- Increase in fat deposition

Second trimester-
1)Quickening(feeling of life)- You finally digest you are in final year and realize that’s life for next 6months
2)Progressive enlargement of lower abdomen and in fundal height- Attributed to fat accumulation
3)Active fetal movements felt and FHS heard- All those weird feelings and noise in the abdomen due to anxiety and anticipation of exams

Third Trimester-
Same things continue with increase in abdominal size and increase in those weird feelings
Abstinence- It is very important to stay away from those frequent movies, outings with friends and families- may lead to abortion and miscarriages(explained later)

Normal Labour- Period during exams
A very crucial phase
1)Oestrogen progesterone and many other hormones are in a state of imbalance- so is the hormonal status of all the exam-going students
2)uterine contractions are at its peak and Bleeding control measures initiated- Contraction of all the interossei and lumbricals, and decreased blood flow to the brain
3)Labour pains- Unbearable tension, stress and anxiety accompanied by pain radiating to all parts of the body. Resistant to all types of anesthesias

1st stage of labour
Just like preparation of birth canal for expulsion, it’s the final revision by the students

2nd stage of labour
Mechanism of labour-                                                          Process of examination
i)Engagement                                                                          i)Medicine 1
ii)Increasing flexion and internal rotation                                  ii)Medicine2
iii)Crowning                                                                           iii)Surgery 1
iv)Delivery of head                                                                 iv)Surgery 2
v)Restitution                                                                           v)Obstetrics
vi)External rotation                                                                 vi)Gynecology
vii)Delivery of shoulders and trunk                                          vii)Pediatrics
‘Descent’ is a continuous process.
‘Episiotomy’ are the copying efforts we put in to ease the process
‘Bearing down efforts’ are required throughout this period

3rd stage of labour
It is painless. Ends with the happiness it brings of coming out as a Graduate doctor

Complications of pregnancy
1)Abortions and miscarriages- are all the unlucky events due to lack of studying. But some threatened abortions manage to continue till term. So never lose hope
2) Antepartum and postpartum hemorrhage- several episodes of crying during the 9 months and post results due to an amazing score or disappointing marks.
3)Hypertensive disorders—Common after 20th week with actual realization that 4 months are over, just 5 months for exams
4)Postterm pregnancy, Prolonged labour, Obstructed labour-Caused due to certain unfortunate events(lets not blame anyone, its destiny), that delays the outcome by 6 months
5)Cephalopelvic disproportion- Time-syllabus disproportion, is a serious disorder which causes stressful conditions of students and all deliveries at the end suffer( you just cant finish the syllabus before exams)
6)Malpresentation and malpositions-Confused and lost state of mind is not uncommon in early months. By 34th week, normal positions are gained by 90%. Some3% manage a normal delivery, even in their confused state

Operative Procedures-
Instrumental delivery and cesarean section- Are all the special efforts of our professors to push us or in fact pull us through, to their side i.e. becoming a doctor

*So my final year friends, all I have to say is just tolerate the pains, difficulties, troubles for a few more months, the fruits are definitely worth our efforts.

Ps:My special thanks to D.C Dutta( author of textbook of obstetrics) for inspiring this ‘in depth’ analysis and Pankti Haria, my co-analyst in this path breaking research.

A DAY I DIDNT WANT TO STUDY

With all those MBBS books around in my room,
I sat for 2mins thinking for myself and had this bundle of thoughts, in my brain, quickly zoom.
That moment, I thought it probably be better if I just took up to work with the broom.

I felt like those cats, dogs and cows on the streets have a better life than me,
Atleast they can eat and sleep for hours together and when needed away from their dangers, flee.

Those birds high up are truly lucky, I thought,
They are dumb as ever, but yet at the top of the world, with life completely sought.

The children down on the grounds, playing stupid games like catch-n-cook and hide-n -seek also seemed so much fun,
To get away from books, I wouldnt mind standing for an hour, without a reason in the hot sun.

Simply lying in the bed and staring at the ceiling all day, was something I wished to do,
I wouldnt mind spending extra time doing nothing, even in the loo.

On any other day the newspaper would be so boring,
But on this one day, even news was more interesting than my MBBS books with info and info just pouring.

That day I would actually love to help Mamma cook,
But I was wondering about mamma's shocked and surprised look.

Then thought of a hated job, arranging my cupboard and setting all things in place,
It was surely better than Harrison and Bailey that I would otherwise have to face.

Suddenly I was awakened from my fascinating day-dreaming, by a phone call,
A very boring, pakao friend; but for hours I spoke, as didnt want to go back to my books at all.

I wished to watch the saas-bahu saga that day,
Atleast that would be more entertaining than reading about that fever, Hay.

Finally I thought I shall sit, analyze and evaluate why was I exactly here,
I realized I had no time for that and absolutely no way to escape, so back to my books I went, with the exam fear.

PS- This is specially for all my dear friends who are all set and ready to pounce on me with their comments and abuses. This whole poem was just true for a day, actually just a few hours. At the end I am and will always be too happy and proud to be a doctor. :-) :-)

LIFE AT THE KATTA

‘Katta’ is the most happening place in RGMC
A place which has definitely been a part of each RGMCite at some point of time

For the latest updates and news in college, the katta is where you have to go,
One of the gossip masters is always in and around,
They always put up an interesting show…

And while our pretty girls sit at the katta and have their chat,
Our famous cricketers of college are all ready at the terrace to bat.

With so much happening here, how can one miss the love and romance?
While some are always getting into problems or solving some fight.
There are some others, who don’t let go this wonderful chance,
They have put aside all the sadness and have set their lives just right.

When the library gets all boring and hot,
It’s the katta where you can give padhai a new shot.
With cold winds and pleasant air around,
It’s this new atmosphere and mood for studying that’s found.

And if you ever find yourself short of friends
Don’t you worry we have many sweet doggies roaming about in all the corners and ends
They’ll accompany you to the lecture hall and even the loo,
But at times they are irritating they dirty places around with their poo poo.

Even though we are screamed and shouted at,
For eating at the katta, while standing in the sun;
It’s this place all of us enjoy, the ambience around is absolutely fun.

May be just some romantic talk or the signal in the library that’s horribly bad,
Katta is the perfect place to be,
You can talk and talk for hours till the bill reaches your dad.

Katta has its maximum significance in the 2nd year of college,
It’s the waiting room for those long long patho-micro pracs,
And for all those moments you bored of the books in the racks.

Finally, it’s this little place, that’s adds all the excitement to our otherwise not so very interesting days.    JJ

MY NON-BIOLOGICAL SIBLING

We fight worst than enemies do,
Say mean things like please go, I don’t need you.
And then cover up for each other too,
With lots of affectionate words, so that we start up a new.

And we when we want to show we care,

Dadagiri to the core I swear,
“Don’t act smart cause I love you too,
There’s nothing much without you, I can do.

Then it’s like a happy end,
Shining stars in the sky, till the next morning has send.
Another topic for us to fight,
So serious that we want to slap each other hard and tight.

But yet don’t know what suddenly calms us down,

We laugh at each other like a mad clown.
Then repeat the statements we have just said,
Think about where we started and where it lead.

I’m glad no one hears us when we scream,

They would all wonder whether we yet in a dream.
For such rude things to each other we say,
Sorrys and love yous follow our way.
God bless us we have finally sought out our fight of the day.

There’s no one who can cross our degree of being mad,

We hate to show that we are ever sad.
We are capable of turning the world upside down,
Together we can make them smile, who didn’t know anything but to frown.

Its said life’s god’s greatest gift,

But I have received much more.
Mamma-pappa, di-jiju, a lovely family and finally you my bhai,
Are people above life, I adore.

Our bonds too strong,

I’m confident it will go really super long.
Yet don’t know where it had all begun
But I have to mention having a brother like you, is lots of fun.

HAPPY BIRTHDAY JIJU

Long back on 15th feb 1984,
Was born my jiju, tall, dark and handsome, someone you’ll definitely adore.

Jiju’s pjs are a true disaster,
But its better you laugh at them, so that they pass and end faster.

The funniest is when he’s on the bike,
It feels like there is no engine attached and he’s  on a hike.

Jiju’s road sense is worth a talk,
When he’s alone, it seems he’s been left in a maze merely to have a walk.

He’s a chocolate fan, one I have never seen before,
Even two servings of desserts are not enough, he always wants more, more and much more.

Jiju is wonderfully phenomenal at his writing skills,
And its an advantage for sure, because on all the special occasions he is saved from paying long shopping bills.

Dare you make fun of him, he really sings well,
Wait, wait, wait, that’s not me, but what his beautiful wife says.
The second time I heard him sing, I laughed and laughed and off the chair I fell.

But he’s truly a gem of a person, with his own special charm,
Can make you smile in a split of a second, there’s no one on earth he can ever harm.
